“The Word 2007 Equation editor will become common to all applications, replacing MS Equation 3.0.”

Not exactly true. What IS true is that the OMML Equation Editor (i.e., the Word 2007 Equation Editor) will become common to all applications.

What is NOT true is that it is “replacing MS Equation 3.0″. The Microsoft Equation 3.0 editor is still in Office 2010, accessible in the same way as always — Insert > Object > Microsoft Equation 3.0. During the install sequence, it’s listed in the “Other tools” section.

Beware of upgrades. If you are happy with the performance of what you have, don't risk permanently losing some of the features you use.

To clear this you need to tweak your Autofill preferences. Go to Safari –> Preferences and click on the Autofill tab.
It's the "Other forms" that you want to check out. Click on "Edit…" next to that field and you'll find a long list of different domain names that have forms your browser is sneakily tracking:
In that choose "Remove all".
Your browser also has a cache (history) of actual pages you visit. If you want to get rid of those too, then you can't be selective, you'll have to erase all of your browser cache. That can be pretty easily done by choosing Safari –> Clear Cache…. You'll get a warning dialog, Proceed with this action and you should be clear.

The formatting toolbar should show up whenever you open the Compose page, the Reply page, and the Forward page. If it doesn't, it is because the page and all its controls didn't load properly. Press F5 to reload the page and its controls. I find I sometimes have to do this a few times to get the page to open properly.
