!4 ga is only permitted on a 15 amp breaker for lights or a single receptacle. 12 ga is a 20 amp breaker for general purposes. For certain other permitted use such as motors on dedicated circuits, the rating may be different.

There is no 'valid' as no one requires you to do this.
If someone is requiring it – like the state – then the form is actually provided by someone else.
Typically, this would be a state mandated form, that you get with your Workers Compensation insurance policy.
It's not something you type up yourself. So if it's something you ARE typing up yourself, there's no "validity". Whoever you want to put on it, you can.
